Abstract

악관절조영술과 자기공명영상장치를 이용하여 정상인 5명과 관절잡음이 있거나 관절걸림
을 갖는 악관절기능장애환자 5명의 악관절을 대상으로 영상을 채득하였다. 영상채득은 과두
에 대해 개, 폐구시 CSMEMP, MPGR 고주파 펄스방식에 따라 시상사면의 여러면을 주사
한 뒤 악관절조영술상과 비교분석하여 다음과 같은 결론을 얻었다.
1. 관절잡음을 갖는 악관절기능장애자의 경우, CSMEMP, MPGR 고주파 펄스 방식에 의
한 시상단면상에서 악관절조영술상에 비해 폐, 개구위치시의 전방변위된 관절원판이 직접
관찰되었다.
2. MPGR 고주파 펄스방식에 의한 상에서 폐구위시 부분적으로, 개구시에는 관절와와 과
두후방부에서 관절강액이 뚜렷한 에너지 신호를 내어 삼각형의 흰 영상이 후방인대의 회색
상에 의해 구분되어 나타났다.
3. CSMEMP고주파 펄스방식에서 개구위 시상사면상에서 과두후방에서 관절강액이 회색
으로 나타났다.
4. 과두와 관절원판 그리고 관절강액의 상은 각단면상에서 다양한 구조관계를 나타냈다.

The Internal derangement of temporomandibular joint disc was evaluated by using
magnetic resonance imaging and arthrogram in 5 patients having reciprocal clicking or
locking and in 5 normal subjects.
Parasagittal multisections on both closed and open mouth were serially obtained by
using a 1.5 Tesla MR system and surface coil pith CSMEMP, MPGR. MR images
obtained were analized by correlating with images of arthrograms.
The obtained results were as follows :
1. Displaced meniscus was clearly delineated as dark structure on MR images other
than on arthrograms of closed mouth view of patient having clicking or locking.
2. The deltoid white images of synovial fluid were identified in the glenoid fossa and
on the posterior surface of condyle on open mouth view and partly depicted on closed
mouth view, of parasagittal sections by MPGR.
3. The greyish image of joint fluid was identified on the posterior surface of condyle
on the open mouth view of parasagittal sections by CSMEMP.
4. The structural relationship among condyle, meniscus, and fluid showed the variety
of images owl each parasagittal view.
